Job description

Candidates must have experience in the healthcare industry and have all required certificates up-to-date in order to be considered. Flexible schedule to work various shifts including nights and weekends; working at all 5 boroughs. Duties include the following but are not limited to:

  • Providing one to one services to individuals with intellectual and developmental disabilities in accordance with their Habilitation Plan of Care (this is done through skills training and development, socialization, recreation and leisure activities, and utilization of community resources)
  • Serving as an advocate for the wants, needs, and rights of the individual
  • Accepting responsibility for the safety and well-being of the individuals in their care in all environments where services are rendered
  • Accompany individuals to appointments as determined in the plan of care or authorized by the Home Care Department
  • Observing and following through on established emergency notification procedures in a manner which ensures the safety and welfare of individuals at all times
  • Providing services according to scheduled hours authorized by Staffing Coordinator
  • Providing relevant information concerning the individuals to the Home Care Department to assist in the creation
  • Maintaining records including the individual's file, plan of care and program and progress reports
  • Completing daily service records in accordance with departmental policies and procedures
  • Adhere to all company policies and procedures regarding timely notification and recording of events and incidents involving the individual

What you need to have

  • Attention to detail and good communication skills
  • Must have physical agility with repetitive occurrences including but not limited to: walking, running, bending, stooping, climbing stairs, kneeling, and crouching, crawling, balancing, reaching standing, sitting and twisting, lifting greater than 50 pounds repeatedly throughout the work day
  • Must be able to maintain the care and use of supplies and equipment
Prior experience & qualifications
  • Attend and successfully complete all trainings requested by the Home Care Department, including all annual compliance trainings
  • * At least 1 yr of relevant experience in the healthcare industry and 1+ years of work experience in a Human Service field or Direct Care field (e.g. childcare, care for elderly, psychiatric care, shelter work, etc
  • Six (6) months or more experience working with people with intellectual & developmental disabilities
  • The position requires six (6) months of work experience with college credits/degree or certifications (e.g. HHA, PCA, CNA)
